Good question. May I suggest two possible solutions? 1) Make a symbol with a third pin (Pin 3) which doesn't have to go anywhere electrically but is for the thermal pad; Or, 2) add a graphic to the symbol to represent the required heatsinking and have that connected to Pin 3. Then in the footprint, make the heatsink and thermal vias all assigned to pin 3. That keeps the schematic and PCB in sync and makes routing and DRC checking simple. I should say, you don't have to do this. You can just add a third pad to the footprint and not have a pin on the symbol but then the DRC checking might be a bit more confusing.
