Basically, you can do this with a comparator.
To make the transitions clean you probably need to add some hysteresis.
To design a comparator circuit do do this, you need to answer the questions below.
1. What are the high and low level voltages that you want to switch at? In other words: what input voltage represents a logic low and what input voltage represents a logic high?
2. What is the resistance of your signal source?
3. Also, what speed is the input voltage changing at (frequency, PRF, rise/fall times)?
4. What are your output logic levels (I think you wanted 0V and +5V)?
5. What output rise and fall times do you need?
6. What power supplies do you have available?
